Transitional Youth Psychiatry Clinic
The Transitional Youth Psychiatry Clinic provides a psychiatric assessment with ongoing support for emerging youth aged 16-24 as they transition from the child and youth mental health sector and into the adult mental health system. This program will help to empower, assess barriers and support system navigation to ensure an appropriate and ongoing connection as they transition into their adult lives.
This program is for individuals who will have had mental health and/or psychiatry supports through HDGH’s Regional Children’s Centre (RCC), Maryvale or the Teen Health Centre.
Priority will be given to those emerging youth with high needs, specifically, those that require multidisciplinary supports beyond what their family doctors can provide. They will be followed by the clinic physician until they are able to be connected to an adult psychiatrist for ongoing care.
Referrals
Referrals to the Transitional Youth Psychiatry Clinic can be made by HDGH’s RCC, Maryvale or the Teen Health Centre.
